Example letters of consent

After submitting your child's Deed Poll application, you need to fax or post a letter of consent to us, examples of which are given below.  Our fax and postal address details can be found on each example letter page.

If you have Microsoft Word for Windows versions 2002, 2003 or 2007 on your computer, you can download an interactive Word template that will make it easy to produce the appropriate letter of consent.  Look out for this download icon in the example letter templates Look out for this icon to download an interactive Word document for your letter of consent.

Example letters

Letter 1 Sole parental responsibility
This letter is required if you have sole parental responsibility i.e. no other person has parental responsibility for your child.
Letter 2 Sole parental responsibility, spouse deceased
This letter is required if you have sole parental responsibility and your spouse is deceased (and no one was appointed a legal guardian in your spouse's will).
Letter 3 Joint parental responsibility
This letter is required if you have parental responsibility with someone else, for example, your spouse, and you both consent to your child's name change.
Letter 4 Joint parental responsibility, absent father
This letter is required if you have joint parental responsibility with the father but the father is absent and his whereabouts is not known.
