How to apply
Apply online

You do not need a debit or credit card to apply online.  You can submit an application and then send us a cheque or a postal order.

Click on a link below to apply online
Apply for an Adult Deed Poll For changing the name of a person who is at least 16 years of age
(or is approaching their 16th birthday).
Apply for a Child Deed Poll For changing the name of a young person who is under 16 years of age.  The applicant must have parental responsibility for the child.
Apply for a Family Deed Poll For changing the surname of a family (at least one parent and a child who is under 16 years of age). Note: the parent(s) can also change their forenames if required. If any son/daughter is 16 years of age or over, you also need to submit a separate adult application.
