How to apply
Apply by telephone

Helpline hours:
9am to 6pm Monday to Friday (closed bank holidays)
9am to 1pm Saturday

If you can pay by credit or debit card,
you can apply by telephone for an Adult Deed Poll or for a Child Deed
Poll. A Deed Poll Officer will complete an application form for
you and answer any questions you have. If applying for a Child
Deed Poll, you must have sole parental responsibility. If you
have joint parental responsibility, you must apply either online, by
post or in person.
